iQ IMAGE Order Form

Here you can submit purchase orders or provide license information. If you have your customer number (from subject line or quotes from iQ IMAGE) available, it can help reduce data entry. Please ensure all mandatory fields are completed.
The form is structured as follows:

  1. Your Data: This section collects your information, which will form the basis of the invoice.
  2. End User Information: Details of where the installation will take place.
  3. Product Selection: Choose the relevant product groups (PACS / CLOUD) and enter the requested product data.

If license information is already available, please provide it in the form to avoid delays in activation. Note that there is only one upload field for license files. For any additional comments regarding products or requirements, please use the general comment field.
